HEMS Pilot Jobs (2026): Air Medical Helicopter Pilot Pay, Hour Minimums and Who’s Hiring

Short answer: As of Oct 26, 2026, the two largest U.S. air medical operators had about 250 open helicopter pilot postings between them. Most Air Methods postings list a base scale of $82,900–$93,251 before geographic adjustments and stipends. Global Medical Response (Air Evac Lifeteam, Med-Trans and REACH) posted ranges from $88,759–$97,635 to $157,322–$173,069, with a median of about $106,500–$117,200. Nearly every posting asks for 2,000 hours total time, 1,000 hours helicopter PIC, a helicopter instrument rating and night experience. That is well above the FAA floor of 500 hours for Part 135 VFR PIC. The usual schedule is 7 days on, 7 off, on 12-hour shifts.

What a HEMS pilot job looks like

Helicopter air ambulance (HAA, or HEMS) pilots fly patients and medical crews from scenes and hospitals under Part 135. Air Methods says its pilots typically work a seven-on / seven-off rotation with 12-hour shifts and log 200–300 flight hours a year (Air Methods pilot careers). 125 of the 150 Global Medical Response (GMR) helicopter pilot postings we checked also advertise a 7/7 schedule, and many include on-duty housing (Global Medical Response careers).

Air medical is the biggest single source of helicopter pilot jobs right now. WingmanPro’s October 2026 snapshot counted 418 air medical openings out of 2,020 pilot jobs (20.7%), with Air Methods (157) and GMR (149) at the top (WingmanPro (Oct 9, 2026)). WingmanPro is a job aggregator, so treat its numbers as its own snapshot. The counts below come straight from each employer’s careers site.

Hour minimums: FAA floor vs what employers ask

The FAA minimum to act as Part 135 PIC under VFR is a commercial certificate and 500 hours as a pilot, including 100 cross-country and 25 night (§135.243(b), 14 CFR 135.243 (eCFR)). For helicopter air ambulance work, §135.603 adds that the PIC must hold a helicopter instrument rating (or a helicopter ATP not limited to VFR) (14 CFR 135.603 (eCFR)). The big operators hire well above that floor:

Employer programPublished minimums (checked Oct 26, 2026)
Air Methods, rotor-wing VFR (Air Methods pilot careers)2,000 TT with 1,500 in category; 1,000 PIC in category; 500 rotor-wing turbine; 200 cross-country (50 night); 100 unaided night PIC (100 NVG can substitute for 50 of those, but at least 50 must be unaided); 50 actual or simulated instrument in category (sim time only in a Level D)
Air Methods, rotor-wing IFR (Air Methods pilot careers)2,500 TT with 2,000 in category; 1,000 PIC in category; 500 cross-country; 75 instrument (at least 50 in aircraft, 20 actual)
GMR (Air Evac Lifeteam, Med-Trans, REACH) (Global Medical Response careers)Commercial rotorcraft; helicopter instrument rating; Class II medical; 2,000 TT; 1,500 helicopter; 1,000 helicopter PIC; 500 turbine helicopter; 100 night unaided, or 50 unaided plus 100 aided. REACH SPIFR postings add 75 helicopter instrument hours (50 in aircraft)

Of GMR’s 150 helicopter line and float pilot postings, 145 list 2,000 hours total time and one lists 2,500. 148 list a helicopter instrument rating. Air Methods requires a commercial certificate with an instrument rating (or an ATP), a second-class medical (first-class for some contracts), and flight time in category within the last 24 months (Air Methods pilot careers). Several postings also set a weight limit: GMR’s REACH SPIFR postings say 230 lb with flight gear, and an Air Methods Bell 407 posting says 250 lb. What HEMS pilots are paid

HEMS pilot base pay ranges: Air Methods $82,900–$93,251; GMR lowest $88,759–$97,635, Air Evac median $106,511–$117,162, Med-Trans median $110,505–$121,555, REACH median $115,387–$131,807, highest $157,322–$173,069
Published base ranges on open postings, checked Oct 26, 2026. Sources: Global Medical Response careers, Air Methods job postings. Bonuses and stipends excluded.

Air Methods: 63 of its 100 open helicopter pilot postings list a minimum of $82,900 and a maximum of $93,251 a year (Air Methods job postings). One Mercy Air EC135 IFR posting spells out the four-step scale: A $82,900, B $86,216, C $89,665, D $93,251. A geographic modifier (“ACCRA”) is not included in base, and that posting lists $91,190–$102,576 at 110%. It adds a $25,000 stipend, a $3,000 IFR stipend, an experience stipend of up to $12,000 and a $40,000 sign-on bonus (Air Methods posting 10529 (Mercy Air, Imperial, CA)). Most Air Methods postings offer a $15,000 sign-on bonus. Air Methods pilots are represented by OPEIU Local 109, and Air Methods says pay is guided by the union agreement (Air Methods pilot careers).

Global Medical Response: 145 of GMR’s 150 helicopter pilot postings publish a base range (Global Medical Response careers). The lowest was $88,759–$97,635 (Air Evac Lifeteam, Denison, TX). The highest was $157,322–$173,069 (REACH SPIFR, Ukiah, CA). The median range was $106,511–$117,162. By brand, median ranges were about $106,511–$117,162 at Air Evac Lifeteam (74 postings), $110,505–$121,555 at Med-Trans (48) and $115,387–$131,807 at REACH (21). Many GMR ranges include a stated geographic modifier, most often 10% to 30% (up to 55% on one posting). 113 postings list both a $15,000 sign-on bonus and a $40,000 retention bonus after 3 years.

Watch the “total potential” headlines. Some GMR postings advertise “$220,000 Total Potential” or “$280,000 Total Potential,” but those include bonus potential of up to $85,000 or $110,000. The base ranges on those same postings are $125,260–$137,789 and $157,322–$173,069. WingmanPro’s snapshot put the median posted air medical pay at $96,883 across 224 listings (WingmanPro (Oct 9, 2026)).

Safety: this month’s NTSB report

On Sep 30, 2026, a REACH Air Medical Services EC135 crashed into the water shortly after a night departure from Pebbly Beach on Catalina Island, bound for Torrance, CA. The pilot, a flight nurse and the patient died. The flight paramedic was seriously injured and a passenger had minor injuries. The NTSB’s preliminary report says the helicopter reached about 90 ft MSL before descending, and that another operator’s medical helicopter, which departed about 45 seconds earlier, also contacted the water before climbing away. The preliminary report doesn’t state a cause (NTSB preliminary report WPR26FA363). Dark-night overwater departures are part of the job. When you interview, ask about NVG use, IFR capability and how the base handles weather.
